Training Overview

MNASE BASKETBALL TRAINING CONTRACT, WAIVER, AND TERMS & CONDITIONS

(PLEASE READ BEFORE SIGNING AGREEMENT)

It’s GAME TIME!!! Thank you for your decision to unleash your inner basketball MENACE (MNASE)!

MNASE Basketball League LLC programs include season competitive AAU play as well as Training and Development of athletes. This agreement is to cover the TRAINING SESSIONS of athletes who would like to take their game to the next level. Athletes that are interested in competing in the AAU season must visit us online at www.mnasebasketball.com for current programs.

Together we will focus on building up your weaknesses so that that they become a strength. We expect all trainees to train yourself on what we work on outside of our sessions, this will be your essential key to long-term success. Great players are not built in a few hours of work a week, they remain optimistic and durable through hours and hours of hard-work, failures, disappointments and adversity to finally become successful. Remember everyone does not have the same concept of success.

I agree and consent to the following: I am voluntarily participating in the MNASE Basketball League Training Program conducted by MNASE Basketball League LLC. I recognize that the program requires physical exertion that may be strenuous at times and may cause physical injury and I am fully aware of the risks and hazards involved.

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any training program. I represent and warrant that I have no medical condition that would prevent my participation in MNASE Basketball League training program. I agree to assume full responsibility for any risks, injuries or damage know or unknown which I might incur as a result of participating in the program. Such injuries may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, fatigue, injuries to knees, injuries to back, injuries to foot, or any other illness or soreness, including death. I knowingly, voluntarily and expressly waive any claim I may have against the MNASE Basketball League and any facility organization used by MNASE Basketball League LLC for injury or damages that I may sustain as a result of participating in the program. I, my heirs or representatives forever release waive, discharge and covenant not to sue the MNASE Basketball League LLC and/or any facility or venue used by MNASE Basketball League LLC for any injury or death caused by their negligence or other acts. I have read the above waiver and release of liability and fully understand it contents.

By signing, you accept full responsibility for your health and well-being and you acknowledge, understand and accept that no responsibility is assumed by the facilities and/or venues, MNASE Basketball League LLC, its trainers and/or coaches.

Setting Up Trainings

STEPS TO SET UP YOUR TRAINING SESSIONS:
  1. CLICK ON THE TRAINING SCHEDULER LINK TO THE LEFT TO SELECT THE DATES OF YOUR TRAINING.
  2. DOWNLOAD THE TRAINING CONTRACT AFTER YOUR SELECTED DATES.
  3. FILL OUT THE TRAINING AGREEMENT AND CONTRACT BY SELECTING THE PROGRAMMING AND FILLING IN ALL SCHEDULED DATES.
  4. SEND THE COMPLETED FORM TO [email protected]
  5. RECEIVE YOUR APPROVED TRAINING AGREEMENT AND START TRAINING!

Individual Training Options and Details

PERSONAL TRAINING OPTIONS

Personal Training Sessions are 90 minutes long and do NOT include the usage of the Dr. Dish CT. Personal Trainings Sessions will be one on one training sessions with the athlete and the trainer. No one else is allowed to attend training sessions unless they are recording the sessions for the training, with the equipment provided by the trainer.

PAY AS YOU GO: 1 Session Billed at @ $45 PER SESSION

Sessions will not be carried over, and expire at the end of the session time scheduled. Sessions must be paid for at time of arrival. Sessions must be scheduled in advance to secure spot. Pay As You Go Sessions are subject to a deposit of $15 prior to scheduling. Deposit will go toward the cost of the sessions. Deposits are forfeited if sessions are missed.

PREPAID WEEK: 2 Sessions Billed @ $85

Sessions are paid/scheduled in advance. Sessions subject to a $25 Deposit, this deposit will go towards the cost of your sessions, unless sessions are cancelled and/or no-shows, the deposit will be forfeited. Any unused sessions will be carried over for 7 days from purchase, deposits are forfeited at the end of the 7-day roll-over period.

PREPAID WEEK: 3 Sessions Billed @ $125

Sessions are paid/scheduled in advance. Sessions subject to a $45 Deposit, this deposit will go towards the cost of your sessions, unless sessions are cancelled and/or no-shows, the deposit will be forfeited. Any unused sessions will be carried over for 14 days from purchase, deposits are forfeited at the end of the 14-day roll over period.

PREPAID MONTH: 17 Total Sessions Billed at $500 (SAVINGS OF OVER $175)

All sessions are paid/scheduled in advance. Sessions subject to a $80 Deposit, this deposit will go towards the cost of your sessions, unless sessions are cancelled and/or no-shows, the deposit will be forfeited. Client will be charged before the first training session. Any unused sessions expire 28 days from the scheduled evaluation date. Prepaid monthly sessions plans may be split into 2 bi-weekly payments or 4 weekly payments. 

Dr. Dish CT Training Details

DR. DISH CT TRAININGS (“DDR”)

Pay As You Go Rates: $35/30 minutes or $60/Hour

In person sessions paired with Dr. Dish CT Programs. The Dr. Dish CT Machine will provide stats to be reviewed by the players as well as access to the Dr. Dish CT app to create a profile and compete on their leaderboard.

7 Workout Skills Camps: Based on Skill Level (See Skill Level)

Skills Level One: $85

1 Hour & 45 Minutes

Includes Agility Warm-Up, Ball-Handling, and Shooting Drills.

Skills Level Two: $115

2 Hours & 20 Minutes

Level One Plus More Complicated Ball-Handling Moves and Increased Reps.

Skills Level Three: $140

3 Hours & 10 Minutes

Level 2 Plus, Increased Reps and Higher Level of Condition.

 

Create A Plan: 30 minute Minimum, billed at current rate above.

                This option is for participants and/or parents that would like to have a plan created by using the mobile app and/or the trainer customizing a plan for the participant. Plans created using the mobile app will be accessible by the DR. Dish CT once the player inputs their information into the machine. Players requesting to create their own plan will be required to provide their phone number and four-digit pin to the trainer.

2K Shot Club: $198 (Save Over $55)

                The 2,000 Shot Club Program includes 6 workouts that focus on shooting from all locations on the court. The player will progressively master stationary and shooting on the move over the course of 2 weeks. Players will schedule 3 sessions per week, each session will be 60 minutes long, this program is about 4 hours and 15 minutes long.

4K Shot Club: $340 (Save Over $160)

                The 4,000 Shot Club Program includes 12 Workouts that focus on shooting from all locations on the court. The player will progressively master stationery and movement shooting over the course of 4 weeks. Players will progressively master stationery and movement shooting. This program will also highlight foot work and step-back shooting. Players will schedule 3 sessions per week, each session will be 60 minutes long, this program is about 8 hours and 25 minutes long.

6K Shot Club: $432 (Save Over $335)

                The 6,000 Shot Club Program includes 18 workouts that focus on shooting from all locations on the court. Players will progressively master stationery and movement shooting then move into mastering foot work and step-back shooting. To finish this program, players will sharpen their skills focusing on individual 1v1 shooting moves. Players will schedule 3 sessions per week, each session will be 60 minutes long, this program is roughly 12 hours and 50 minutes long.

8K Shot Club: $510 (Save Over $515)

                The 8,000 Shot Club Program includes 24 workouts that focus on shooting from all locations on the court. The player will progressively master stationary and shooting on the move over the course of 8 weeks, then move into mastering foot work and step-back shooting. Players will also sharpen their skills focusing on individual 1v1 shooting moves and finish this program by working on double move and jab step shooting. Players will schedule 3 sessions per week, each session will be 60 minutes long, this program is roughly 17 hours and 10 minutes long.
